Registration Checklist

REGISTRATION CHECK LIST

2024-2025

• STEP ONE : COMPLETE REGISTRATION

  or packet that was given at school.

• STEP TWO: COMPLETE, SIGN AND NOTARIZE DOCUMENTS

Complete, sign, and notarized all documents that were sent home in this registration packet. Below is a list of the documents.

1. Registration Form ( for students entering 2nd and 6th grades only)

2. School Policies and Tuition and Fee Agreement ( Must be notarized)

3. Emergency Data Card

4. Consent and Authorization Form (Must be notarized)

Immunizations – All students must have a current health and immunization record on file.

First graders must also provide proof of M.M.R Series Immunization.

All seventh graders are required proof of immunization for the following:

• Hepatitis B Series

• Tetanus-Diphtheria Booster

• A second dose of measles vaccine (preferably MMR vaccine)

• STEP THREE: SUBMIT REGISTRATION FORMS

Return all completed documents to the office along with payment.

$250 Registration Fee, per new student.

$200 Registration Fee, per returning student.

Registration Fee includes: school supplies and workbooks.

DISCOUNTS -

$25 discount per sibling for old and new students. 

$25 discount per child can be offered with review on Google.

Please note that the registration fee will not be accepted if the required documents are not completed in full and/or notarized as indicated.
